As for me, I failed and upon later inspection I found an exhaust leak where my cat connects to my midpipe. Would this be the cause of my fail? I have my doubts because it is after the catalytic converter.

An exhaust leak will only cause you to fail if the leak is big enough that your o2 levels are reading high. It has to be a pretty big leak to fail...

On a 91 the O2 is upstream of the cat. It is hard to know what is going on without seeing your emission numbers.
