The MR shift extender, tends to loosen up over time due to using a set screw.

If you can mark where the set screw tries to hook on the shifter, file a groove in it for the set screw to catch

I never had to use the set screw. What I did to achieve the desired position (above), I had to force the shift extender by using a 14mm deep socket with a power handle into the shifter. The MR shift extender is made of aluminum and will easily grind through the metal shifter
